Hospital at Home

For:

Hospital at Home offers you the same level of care as you would receive in a hospital ward, but in the place you call home. This means you can receive vital care from a familiar space of comfort, as long as we can make it safe for you.

About the team

​​​Our team is full of highly qualified specialists, including doctors, nurses, paramedics, therapists and clinical support workers. This is to make sure you get safe, high-quality care.

We base everything we do around your needs – and so your care might include scheduled home visits based on your needs, wearing digital devices that monitor your health when a professional isn’t around and fast access to hospital care if it’s ever needed.

Our aim is to help you recover while allowing you to be as independent as possible – continuing with your daily routines, dressing and eating how you like and staying in control of what you do day-to-day.

Who this service is for

We can usually help if:

  • You are an adult living in our service area
  • You are well enough to be treated safely at home
  • You have been referred by a healthcare professional

We may not be the right service if:

  • You need emergency hospital treatment
  • It's not safe to monitor you at home
  • You’re under 18

Getting help from this service

Professional referral only

This service accepts referrals from professionals, including GPs, only. If you're unsure who can refer you, contact us for advice.

Questions and answers

Do I need a GP referral?
You need to be referred by a healthcare professional. This might be your GP, a hospital doctor, or another member of your care team.
How long will you look after me?
We can care for people for up to 14 days. The exact time depends on your situation and how you are recovering. We will talk this through with you, so you know what to expect.
