This page provides guidance for acute hospital teams on how to refer patients to BSW Community Health services. It explains the different referral routes, including urgent pathways and the Single Point of Access (SPA), to help ensure referrals are made quickly and to the most appropriate service.

About this service

This service supports acute teams to refer patients into BSW Community Health, including both urgent and routine pathways. Urgent referrals are managed through direct clinician-to-clinician discussion to enable rapid triage and timely care, while all other referrals are coordinated through the Single Point of Access (SPA) to ensure patients are directed to the most appropriate service.

Referral pathways

If you need help locating, completing or submitting a referral, the SPA team can provide support (please call the SPA on the telephone number at the top of the page).

Please make all other referrals via the BSW Community Health Single Point of Access (SPA). All referrals should be sent to the appropriate email address below.

Please call our Clinician-to-Clinician line on 0300 247 0065 (Option 2) to talk to a triage clinician about making an urgent referral.   

Do not send any referrals requiring Urgent Community Response (UCR), or that are for Hospital at Home (H@H).  

Who this service is for

We can usually help patients who are:

  • The patient is being referred from an acute setting
  • The referral can be managed through SPA
  • Community-based care is appropriate

We may not be the right service for patients if:

  • The referral is urgent and needs clinician discussion first
  • The patient requires Urgent Community Response
  • The referral is for Hospital at Home
  • Immediate clinical decision-making is required
