Our Community Diabetes Service provides specialist support for patients with complex needs caused by diabetes. The service helps you to understand, monitor, and manage your own diabetes, so you can take control of your health and your day-to-day life.

About the team

The team’s nurses are specialists in diabetes. We can advise you, help you learn about your condition, and offer remote consultations if you need them.

We also work closely with you, your GP, and other healthcare professionals to help support you better.

We can usually help if:

  • You’re registered with a GP
  • You’re in the region covered by this service
  • You need help monitoring or managing diabetes

We may not be the right service if:

  • You aren’t registered with a local GP
  • You haven’t been diagnosed with diabetes
  • You live outside the area covered by this service

What is Diabetes Structured Education?
Structured Education Programmes are like mini-courses you can do in your own time. They help you understand how diabetes affects your body. This is vital to help you take control of your own health and avoid situations that could harm you. Some programmes are online and can be completed in your own time, such as My Type 1 Diabetes and The Healthy Living NHS Programme (for Type 2). Face-to-face courses are also available if you are referred by your GP.
What do I do if I lose my insulin pen?
Contact your GP and ask to reorder an insulin pen from your repeat prescription.
What should I do if my glucose meter is faulty?
If your meter stops working, check your manufacturer and call their helpline. The issue can be due to battery failure, and your manufacturer can often supply a replacement.
