Long Term Condition Management
About the team
We are an Allied Health Professionals–led service, including an Occupational Therapist, Respiratory Physiotherapist, Clinical Psychologist, Exercise Instructor, and Speech and Language Therapist. Once we receive your referral, we aim to offer you a comprehensive assessment within 6 weeks. This may take place face to face, by telephone, or virtually, depending on what best suits your needs.
During your assessment, we will explore your medical symptoms, discuss the impact on your daily functioning, help you set personal goals, and develop a tailored rehabilitation plan. We also have access to a multidisciplinary medical team (MDT) for clinical advice and reassurance where needed.
We support people experiencing breathlessness, cough, dizziness, fatigue, brain fog, deconditioning, low mood and anxiety, vocational rehabilitation and employment support, dietary advice and voice and swallowing difficulties. Rehabilitation can be provided in clinic, at home, by telephone, or via video, either one‑to‑one or as part of a group, depending on what will best support your recovery.
Who this service is for
We can usually help if:
- You have been experiencing Long Covid symptoms for more than 4 weeks
- You are aged 18 or over
We may not be the right service if:
- Your symptoms have lasted less than 4 weeks
- You require urgent or emergency care
