Children’s Community Nursing (BaNES, Wiltshire)

When your child has life-limiting or complex long-term medical needs, the right support can make all the difference in helping them have the best quality of life.

Children’s community nursing offers specialist nursing care to children with health needs like, oxygen dependency and tracheostomy care. Support can include home and school visits, hospital discharge planning, end of life support, as well as equipment training and education for school staff and caregivers.

About the team

Our team is made up of registered Children's Nurses who are specially trained to care for children and young people with a range of health needs.

We can support your child and family in different ways. You may meet a Community Children's Nurse (CCN) while your child is in hospital, before they come home. We can also visit your child at home to provide care and support.

CCNs work in schools, nurseries and other community settings too. We help staff learn the skills they need to look after your child safely and confidently.

We work closely with families, schools and other healthcare professionals to make sure your child gets the right care, in the right place, at the right time.

Who this service is for

We can usually help if:

  • You have a life-threatening, life-limiting or complex long-term condition that needs nursing support
  • You have an oxygen dependency that needs home or community support
  • You have a cardiac condition that needs home or community support
  • You Need wound care
  • You have difficulty with your airway and needs a tracheostomy, suction, or long-term ventilation
  • You have palliative care needs

We may not be the right service if:

  • Your child does not have complex, long-term care needs
  • Your child needs urgent medical help for a sudden and new condition
  • You live outside the area covered by this service

What support does the children's community nursing service offer?
Our team of children’s community nurses are trained to offer a wide range of support and care. This can include doing blood tests and sleep studies at home to avoid extra hospital visits. Speaking with all of the medical teams involved in your child’s care on your behalf. Care training for families, carers and school staff. Putting plans in place for your child to return home after hospital stays, creating a personal healthcare plan for your child. Supporting young people who are moving from child to adult health services. Supporting end-of-life care where your child is most comfortable, funding and providing vital medical equipment.
What training can the children’s community nursing service offer to carers?
We offer training for parents and carers, as well as school staff who see your child regularly. Our children’s community nurses also give monthly training sessions to early years staff and passenger transport staff. Our training can include seizure management, using epipens or giving emergency injections (like buccal midazolam to stop long seizures).
