Hospice and bereavement services offer compassionate care and specialist support for people living with conditions that can’t be cured, or lived with for long. We also offer support for families, friends and carers.
Our hospice and bereavement care is provided by a few organisations working in partnership. Together, we work with compassion, prioritising what you need and care about at every stage of the journey. Our help might include managing symptoms, emotional support, and end-of-life care, as well as emotional and practical support if you’re experiencing grief or loss.

About the team
Hospice teams include experienced doctors, nurses, therapists, support workers and volunteers – they all focus on providing palliative and end-of-life care that feels personal to you.
A second team of volunteers and specialists offer emotional support and practical guidance to people who are grieving, as part of our partner’s bereavement service.
All the teams work closely with GP practices, community services and hospital professionals so your care all links up, and so families feel supported throughout.
Who this service is for
We can usually help if:
- You’re an adult (aged 18+)
- You’re a resident of BaNES, Swindon, or Wiltshire
- You’re living with a life‑limiting or progressive condition, and you’re likely to be in the last 1000 days of your life
- You need palliative or end‑of‑life care
- You need help managing symptoms, comfort or your emotional wellbeing
- You’d benefit from hospice support – whether at home, in the community or in a hospice
- You’re family or a carer, grieving the loss of a loved one
We may not be the right service if:
- You’re under 18
- You’re not registered with a GP
- Your condition has reasonable hope of a cure
- You live outside the area covered by this service
