Children’s Continuing Health Care (Swindon)

For:

Children’s Continuing Care supports children and young people who have serious and complex health needs, and who have been assessed as eligible for Children's Continuing Care

We work with you to figure out exactly what kind of care your child and their family needs. This may include specialist care during the day or overnight, usually in your home. Our aim is to provide safe, high-quality care that supports your child’s health and wellbeing – while also giving families some time to rest and recharge.

About the team

Our team includes qualified nurses and trained community support workers, who support children with complex needs such as breathing difficulties, seizures and end-of-life support.

We provide care that may take place during the day or overnight, depending on what your child needs. Most care happens in the family home. In some cases, we can also provide support in nurseries, schools or other settings.

We work closely with families and other healthcare professionals to make sure care is safe, coordinated and centred around what your child needs.

This service is provided by Great Western Hospital.

Who this service is for

We can usually help if:

  • is aged 0-18
  • has been assessed as eligible for Children’s Continuing Care or is already receiving a package of care from us.

We may not be the right service if:

  • your child will be better supported by another service (our care navigators will guide you to the right support)
  • you live outside the area covered by this service

Questions and answers

What conditions does the Children’s Continuing Care Service support?
We support children and young people with complex health needs, including long-term breathing conditions or oxygen support, complex seizures, issues that need ventilator support (such as CPAP – Continuous Positive Airway, pressure – or BIPAP – Bilevel Positive Airway Pressure). The need for an artificial windpipe to help with breathing (a tracheostomy procedure), throat or lung blockages affecting their breathing (needing a tracheostomy procedure or suctioning tube – a catheter). Enteral feeding with other complex needs and end-of-life or palliative care support.
How can the service help?
Our team works with you to plan safe and personalised care for your child – as well as support for your family. Support may include assessing your child’s personal care needs (and listening to the family’s views on them), a personal care plan based around what your child needs, risk assessments to help keep care safe at home (such as: during clinical care, when moving your child, and in case of fire). Community support workers trained to meet the needs of your child, medical equipment (we’ll service and maintain it too) and supplies that aren’t usually available on prescription from the GP.
