The Two Page Profile – Getting ready for your child’s hearing clinic appointment
Coming to a hearing clinic can feel unfamiliar. This guide helps you prepare so your child feels more comfortable and the team can understand their needs. You can complete a short profile before the appointment and bring it with you, or send it to the team in advance if you prefer.

Step 1
Before your appointment
Bring the completed profile with you, or send it to the clinic in advance.
If you need help filling it in, or would like it in another format or language, you can contact the team using the details on your appointment letter.
You can also visit the Children’s Audiology (BaNES, Wiltshire) service for a social story with pictures of the clinic, staff, and what your child might see and do.
Step 2
Tell us about you and your child
Start with some basic information so we know who is coming to the appointment.
Tell us:
- Your name and your relationship to your child
- Your child’s name, date of birth and NHS number
- Your child’s school or education setting
- Any diagnoses your child may have
- Any concerns you have about their ears or hearing
This helps us prepare for your visit.
Step 3
Help your child share about themselves
Your child can tell us what matters to them and how best to support them.
They can share:
- What they like to be called
- What languages they use
- How they communicate (for example, speech, signs, or pictures)
- What they enjoy and are interested in
- What they are good at
Step 4
Let us know what might be difficult
It’s helpful for us to understand anything your child finds challenging.
You can tell us:
- Things they don’t like or feel worried about
- How they feel in different situations (for example, new places)
- Anything that might upset or overwhelm them

Share what helps your child feel comfortable
Let us know what helps your child feel safe and settled.
This might include:
- Favourite toys or comfort items
- Familiar routines
- Helpful ways for us to approach or support them
Step 6
Tell us about important people
Let us know who is important in your child’s life, such as:
- Family members
- Carers
- Friends
This helps us understand their support network.
