If you work in education and need to refer a child or young person to our service, this page explains how to do it. You’ll find the information you need to make a referral, including who can refer, what details to include, and how we use the information you provide. Our aim is to make the process simple and ensure children and families get the right support at the right time.

About this service
This will guide you to the right service and request the information needed for triage. You can complete the form yourself or support a young person or their parent/carer to complete it. Young people can also request help directly through the dedicated children and young people’s form.
If you work in Swindon and currently use a referral form supplied directly by the service, please continue using this for now. We will let you know when the online referral route is available in your area.

Who this service is for
We can usually help patients who are:
- The referral is for a child or young person with health, wellbeing or development needs that may be affecting their education
- You are an education professional with a clear concern and can provide enough detail to support triage, with appropriate consent in place
- You are completing the referral yourself, supporting a family to do so, or signposting them to contact the service directly or use the self-referral form
We may not be the right service for patients if:
- The situation requires urgent or emergency care, or input from a more appropriate specialist service
- You are in an area using an alternative referral route (for example, Swindon services using a local form), or the referral does not include enough information or consent
- There are no identified health or wellbeing concerns, or the need can be fully met through existing school-based support
