Filling out online questionnaires for ADHD Assessment

When we're assessing people for possible ADHD, we may use digital questionnaires called 'Conners‑4'. They are quick and easy to complete online and help us gather information in a clear and secure way. Using digital questionnaires means there is no paperwork to return, nothing to misplace, and no need to post anything back. This helps make the process simpler and smoother for families.

We will only send questionnaires after your child has been referred and the referral has been accepted. The questionnaires are an important part of helping us understand your child as part of the assessment process.

They are usually sent shortly before a planned appointment, so the information is up to date and relevant. We will always let you know before any questionnaires are sent, so you know what to expect and when.

Questions and answers

How will you send them?
We will contact you to check that we have the correct email address and to let you know when a link to the questionnaires will be sent to you. The email with the questionnaire link will come from Q Global at q-global@mail.services.pearsonclinical.com, and the subject line will be “Invitation to Complete Questionnaire”. Knowing this in advance can help you spot the email when it arrives. We will ask you to share the school questionnaire link with your child’s school. This should be sent to a member of staff who has known your child well for at least six months. If you are unsure which email address to use, the school office will be able to advise you. If you have any problems accessing or completing the online questionnaires, support is available. We check how you prefer to communicate before sending the questionnaires, and you can contact us at any time if you need help.
When will you send them?
We will be sending the questionnaires out within a few months of your child’s anticipated appointment with us. This is so that the information is current when they have the next step of the assessment.
How long will I have to complete the questionnaires?
We will tell you in the email when the questionnaires need to be completed by. This is usually within two weeks of them being sent. If there is a school holiday during this time, we will allow extra time so that the school has a fair chance to complete their questionnaire. If you are worried that the questionnaires may not be completed in time, please let us know as soon as you can. We are here to help and can offer support to make sure everything can be completed.
Can I complete the questionnaire on my phone?
Yes, the questionnaire works well on a mobile phone or a computer.
What do I put in the ‘ID’ box on the questionnaire?
Please add your child’s NHS number in this box. If you don’t know this, you can find it by looking in their red book, or go to https://www.nhs.uk/find-nhs-number
What do I put in the ‘grade’ box on the questionnaire?
You can add your child’s year group in school in this box, or leave it blank if you prefer
What if more than one teacher knows the young person well?
We usually ask the school to have one member of staff complete the questionnaire. This should be someone who has a good overall understanding of the young person, such as a tutor or key member of staff. If the young person behaves differently in different lessons or settings, this information is very helpful. The person completing the questionnaire can include this on the final page, where there is space to add extra details. We welcome clear examples of how things may vary, such as what works well and what can be more difficult, and when this happens. For example: "The young person is very focused and follows instructions well in PE. In maths lessons, they may struggle to concentrate and can act impulsively, which sometimes leads to warnings." This kind of information helps build a fuller picture of the young person’s strengths and needs across the school day.
Can I make changes after I have submitted?
Unfortunately not. When you press the final submit button, there's no option to make further changes.
