They are quick and easy to complete online and help us gather information in a clear and secure way. Using digital questionnaires means there is no paperwork to return, nothing to misplace, and no need to post anything back. This helps make the process simpler and smoother for families.

We will only send questionnaires after your child has been referred and the referral has been accepted. The questionnaires are an important part of helping us understand your child as part of the assessment process.
They are usually sent shortly before a planned appointment, so the information is up to date and relevant. We will always let you know before any questionnaires are sent, so you know what to expect and when.
