Who this service is for

We can usually help patients who are:

  • You are a health or care professional referring a child, young person or adult for support with health, wellbeing or development needs
  • You can complete the referral through the digital referral platform and provide enough information to support triage
  • You need guidance on the referral process and may contact us by phone or email for advice or clarification

We may not be the right service for patients if:

  • The referral cannot be completed through the digital referral platform or an alternative local referral route should be used
  • There is no clear health, wellbeing or development need that requires input from this service
  • The query is administrative only and can be resolved without making a referral
