Referral Guidance for Primary Care

This guidance is for primary care professionals referring patients to BSW Community Health services. It explains the different referral routes, including urgent pathways and the use of the Single Point of Access (SPA), as well as system-based referral options, to support efficient, accurate, and timely referrals.

About this service

This service supports acute teams to refer patients to BSW Community Health services through a clear and consistent pathway. Routine referrals are managed via the Single Point of Access (SPA), while urgent referrals are coordinated through direct clinician-to-clinician discussion to enable rapid triage and timely care.

For Children’s services referrals in Swindon, we cannot facilitate digital self-referral through the portal for CYP Services in Swindon yet. However, a form download is available below.

When to use this: For making referrals and sending referral-related documentation only. Use the guidance on each referral form which indicates which email to use.

BSW Community Health needs-led referral forms are available on your GP SystmOne unit.  

  • For BaNES GPs referral forms will be located within the ‘HCRG Referrals’ tab. 
  • For Swindon and Wiltshire GPs, referral forms will be located within the A-Z by service or need. 

The email address that the form needs to be sent to is on each form. 

You will receive an automated email reply to the sender to acknowledge receipt of each referral you make. 

Please get in touch with the SPA on 0300 247 0200 if you have any questions regarding referral forms or the referral process. 

Military GPs will have received copies of referral forms that can be uploaded into your own EMIS systems. Please get in touch with the BSW SPA on 0300 247 0200 if you have any questions regarding referral forms or the referral process. 

Referrals for adult patients known to community services can be made directly from GP SystmOne to Community SystmOne.

A button is available on all GP SystmOne units’ favourites section called ‘Simple Request to Community Adult SPA’. Clicking this generates a partially pre-populated electronic referral, with the SystmOne unit, team and type already populated.

Once the patient record is saved, the referral is sent as a task to the BSW Adults SPA SystmOne unit and processed.

If your unit is configured to receive them, you will receive an automated task confirming when the referral has been accepted or processed.

For patients already known to our Integrated Neighbourhood Teams (INTs) and community services, GP practices can continue to send SystmOne tasks to locality INTs to communicate directly about patients known to the teams. 

Who this service is for

We can usually help patients who are:

  • The patient is being referred from primary care
  • The referral is submitted via the Single Point of Access (SPA)
  • The referral follows a standard community pathway

We may not be the right service for patients if:

  • The referral requires an urgent clinical discussion
  • The patient needs an Urgent Community Response (UCR)
  • The referral is for Hospital at Home
  • The referral is sent via email when urgent escalation is required
