Children’s Continuing Care supports children and young people who have serious and complex health needs, and who have been assessed as eligible for Children's Continuing Care
We work with you to figure out exactly what kind of care your child and their family needs. This may include specialist care during the day or overnight, usually in your home. Our aim is to provide safe, high-quality care that supports your child’s health and wellbeing – while also giving families some time to rest and recharge.

About the team
Our team includes qualified nurses and trained community support workers, who support children with complex needs such as breathing difficulties, seizures and end-of-life support.
We provide care that may take place during the day or overnight, depending on what your child needs. Most care happens in the family home. In some cases, we can also provide support in nurseries, schools or other settings.
We work closely with families and other healthcare professionals to make sure care is safe, coordinated and centred around what your child needs.
This service is provided by Great Western Hospital.
Who this service is for
We can usually help if:
- is aged 0-18
- has been assessed as eligible for Children’s Continuing Care or is already receiving a package of care from us.
We may not be the right service if:
- your child will be better supported by another service (our care navigators will guide you to the right support)
- you live outside the area covered by this service
