Heart Failure (BaNES, Wiltshire)

We help you to manage your condition well, so you can feel as healthy and confident as possible in everyday life. This includes making sure your medication is working as well as it can, and that you have appropriate therapies such as specific pacemakers or defibrillators, giving you exercises and advice to help manage your condition day-to-day.

About the team

Our Heart Failure Team includes specialist heart failure practitioners who have advanced skills in supporting people living with heart failure. Together, we work to help you stay as well as possible, and improve your quality of life.

You may also be supported by the cardiac rehabilitation team, which can include exercise instructors, physiotherapists, nurses and practitioners.

We also work closely with cardiology teams in the acute hospitals to manage complex cases.

Who this service is for

We can usually help if:

  • You are registered with a local GP
  • You have a confirmed diagnosis of heart failure made by a specialist.
  • You need support to manage your condition with medication optimisation, education, advice, and cardiac rehabilitation.

We may not be the right service if:

  • You don’t have a heart failure diagnosis
  • You live outside the area supported by this service

Can I bring someone with me?
Yes. You are very welcome to bring a family member, friend or carer for support. We understand appointments can feel daunting, and it can help to have someone familiar with you.
How long is the waiting list?
The waiting list can vary, but we will contact you as soon as possible. If your symptoms get worse while you are waiting, please speak to your GP.
